do a search for vital statics in your state or province you will have to pay to have areplacement for NS Canada it is $25.00 for wallet and $35.00 for long form wich lists parents and everything.Prices and procedure is different for each place. You should make sure you do your search for Vital Statistics in the state or province you were born in. they should also have a contact Phone # if you want them to send you the forms or most places have forms you can print off and mail to their adress.
