death of a relative

My mother has recently passed away. How do I go about filing a death claim for Adelina Santos of Houston, Texas. Where do I get the forms? Who do I need to contact in Houston area? What other information will I need to process this claim? What if the beneficiary (my father) has pre-deceased my mother? Any additional information would be much appreciated. RegardsResponses
